Children arrived with the 1.0 update. The whole system is optional, your spouse never brings it up, and the timing is fixed rather than random.

How does having a child work in 1.0?

Nothing happens automatically. Two weeks after your wedding day, going to bed triggers a dream in which the Great Bird hands you a Mystical Feather. You then have to hold that feather and talk to your spouse yourself — the full sequence is here. Your spouse will never raise the subject, and the studio's own 1.0 FAQ states that the player has full control over whether to have a baby at all.

Who carries the baby?

A second dream offers three ways for the child to arrive. The Great Bird delivering it is the default and is always available. Carrying the baby yourself is also always available, whatever pronouns you chose at character creation and whoever you married. Your spouse carrying it is offered only when your spouse is female.

Whichever you pick, the timing is identical: the baby arrives two weeks after that second dream, and the due date is written on your calendar. The three options compared.

How many children can you have?

One, and only one, per save file. There is no second Mystical Feather, and because 1.0 has no divorce and no way to remarry, there is no route to a different child later either.

Which child you get is decided by which of the twelve marriage candidates you married. Each has a single child with a fixed gender and appearance, in a lighter and a darker skin tone that follows the one you chose for your own character. Your spouse suggests a name and you can change it; the gender and the look you cannot. Every candidate's child is listed here.

Do children ever grow up?

No. They stay babies indefinitely in 1.0, with no further growth stages and no contribution to farm work. You can carry the baby everywhere with no penalty to farming, fishing, mining or combat, or leave it in the crib Landen mails you when it arrives. What that actually means for your save.
